Author ADR-0017, recording the decision to insert LSP Parametric
Equalizer x16 LR (LV2) hosted by jalv as the PracticeEQ JACK
client on the play-along monitor bridge. The ADR covers context,
decision, alternatives considered, consequences, and references
to ADR-0007, ADR-0008, ADR-0011, and ADR-0012.

Update the ADR index in docs/adr/README.md and the root README.md
to include the new ADR-0017 row, following the existing column
format.

ADR-0003 footer deviation (owner-approved 2026-05-23 during the
staleness sweep): the trailing `Refs #<issue>` footer is omitted
because this rig has no issue tracker on record.
